At Excentia Human Services, our Home and Community Habilitation Services support individuals with intellectual and developmental disabilities (IDD) in developing and maintaining the essential skills needed to live as independently as possible. Understanding Waivers and Payment Options

Home and Community Habilitation services are typically funded through Pennsylvania’s Medicaid Home and Community-Based Services (HCBS) waivers—such as the Consolidated, Community Living, or Person/Family Directed Support (P/FDS) waivers. If you’re eligible and enrolled in one of these programs, the cost of services is covered, meaning there is no out-of-pocket expense for approved supports. To get started, contact your local County MH/IDD office or a Supports Coordinator, who will help determine eligibility based on diagnosis, level of care, and functional need. It is typically delivered one-on-one and emphasizes skill-building for greater independence.
  • Community Participation Supports (CPS), on the other hand, emphasize community inclusion, integration, and building relationships with people without disabilities. These services often take place in small groups and focus on engaging in community life, exploring volunteer opportunities, and preparing for competitive employment.
